Advance Care Planning 101

Creating advance care plans can be confusing. There are many forms to complete and decisions to be made. If you are feeling overwhelmed, you're not alone! But where do you start? In this workshop, we'll discuss the many forms, decisions, and preferences involved with advance care planning. This educational event is for anyone seeking to become more confident in navigating this area, including: medical patients, family members, friends, and health professionals. This workshop is not exclusive to those currently making medical care decisions! Anyone seeking more information on the topic is welcome, regardless of your medical journey. Online classes will be held via Zoom and can be accessed through a laptop, desktop, tablet or smartphone. Your chosen device must be connected to the internet, and have speakers. A webcam is optional but recommended, and important if you'd like the instructor and the class to be able to see you. All registered students will be provided with simple and easy instructions for participation.
